Forms & Surveys Widgets

We need a Widget to create a Custom Form for Team Members to fill out or for the Public to fill out. Each Form would need its own special link we could share anywhere we want to. Admins should be able to add as many Fields to the Form as wanted and Create custom Fields if Form Field needed doesn't exist. A Donation and/or Payment Field needs added also and Admins need to be able to add their own Payment Methods they will Accept and the Person filling out the Form can choose the Payment Method they want to make Payment or Donation with from the ones chosen/added by Admins.

We also need the Ability to create Surveys for our Team Members to take. Surveys are very important in the Business World especially. It lets you know where you're doing good and where you're doing poorly. We also need to select a time and date for the Survey to close and prevent multiple votes from the same person but allow each person to change their vote one time per person.
